Office Policies
All visits are by appointment only. During a standard appointment, your physician is able to cover one issue comprehensively or two small issues. If you have multiple issues, please book another appointment.
If you are accompanying a child/relative to their appointment and also have questions for your physician, we ask that you please book an appointment time for yourself.
All of our physicians reserve a number of appointments each day they are in clinic that are available for same day bookings only. These same day short appointments are to deal with a single issue of an urgent nature (for example, sore throat or urinary tract infection).
If all of the same day appointments are booked, you can call the After Hours clinic which runs seven days a week.
It is your responsibility to ensure your health card is valid at all times. You must have a valid Ontario health card to book an appointment and to bring with you at every visit. Expired cards are not accepted. If you do not have your valid health card at the time of your appointment, you will be charged a non-refundable fee for the visit depending on the nature of the appointment.
If you must cancel your appointment, we respectfully request at least 24 hours’ notice. Cancellations or missed appointments without 24 hours’ notice will result in a fee.
The fee for a missed appointment is $50 for a standard appointment and $100 for an extended-length appointment. Recurrent no-shows are very disruptive to our practice. After three no-shows or late cancellations, you may be dismissed from the practice at the physician’s discretion.
We do our best to run on time during the day, and late arrivals make this difficult. If you arrive late for your appointment time, you may not be seen or you may wait to be seen when there is an opportunity, if time allows (at the physician's discretion). If not, you will need to rebook your appointment.

Accepted payment methods include: all major credit and debit cards (in person or over the phone), e-transfer, cheque, and cash.
Due to the volume of results received in the clinic, it is not possible to communicate all normal results to each patient. You will be informed of significant abnormal results and, if required, a follow up appointment will be scheduled.
Very rarely, results are not received by our office. We always encourage patients to access their own results using the various portals available:

Periodic Health Assessments are generally scheduled every 1 to 3 years depending on your age and health status. This appointment is used to discuss health maintenance, appropriate screening tests, and often includes a focused physical exam.
Our goal is to complete all forms in a timely manner and forms are generally not completed during an appointment unless the visit is booked specifically for this reason. There are fees associated with the completion of forms and payment is required in advance. An in-person or phone appointment may also be required to help your physician complete the form.
After a referral is made, the specialist's office is responsible for contacting you to book an appointment. If you have not heard about the status of your appointment within 2-3 weeks of the referral, please contact the specialist's office directly to inquire about the status of your referral. If you do not have the contact details for the specialist office, please contact our office, by email, to receive this information.
Requests for new referrals are by appointment only. Requests for repeat referrals without an appointment will incur a non-OHIP fee of $30.00.
In order to have your prescription renewed, please have your pharmacy send us a renewal request. Once received, you will be given the option to either book an appointment with your physician or pay the $30.00 prescription renewal fee.
Narcotics, sedatives and antibiotics are never renewed by telephone/fax, and require an appointment.
It is important to us that all patients of St. Clair Medical are treated with politeness and respect.
We have a zero-tolerance policy for inappropriate conduct. If any physician or staff member at St. Clair Medical observes disrespectful, abusive, or threatening behavior or language; or if the patient poses a risk of harm to a physician, other patients, or staff, that patient will be discharged from the practice and the patient-physician relationship will be ended immediately.
We ask that you please leave your pet at home during your visit to our clinic unless the animal is a fully trained guide/hearing/service animal.
Ahead of your visit to the clinic, we ask that you please avoid perfumes and colognes, and try to opt for “unscented” or “fragrance-free” products, particularly in lotions and creams.
